Please complete one form per household even if you're included in the current directory to ensure our records are accurate and up to date. Information will be used to update our church records and to prepare our printed church directory.

1.  Email your family photo to info@cottonwoodcommunity.church. Include your household name so we can match your photo with your directory information. 
2. Upload a photo. Maximize size is 8 MB.
3. Have your photo taken at church. Dates to be announced soon. 
4. Use your family photo from the previous directory.

Cottonwood Community Church uses Breeze, a secure church management system to maintain our church directory information. Breeze uses encrypted connections, regular data backups, and password-protected accounts to help safeguard personal information. Access to directory information is restricted according to permissions established by our church administrators. Information collected for the directory will be used for church-related purposes. We respect your privacy and want you to feel comfortable sharing your information.
